What is MDR-TB?

(Part 2)

The Bohol Tribune (TBT):  In our edition last week we started discussing MDR-TB.  We gave the definition of MDR-TB, its causes, dangers and risks, 

Dr. Bryan Cepedoza:  If I may add, let us all remember that inappropriate and/or incorrect use of TB drugs, use of ineffective formulations of drugs (such as the use of single drugs, poor quality medicines or bad storage conditions), and premature treatment interruption can cause drug resistance. And this can be transmitted, especially in heavily crowded areas.

The Bohol Tribune (TBT):  According to data, there are already many people who are sick of MDR TB. How do we know if we have such kind of TB?

Dr. Bryan Cepedoza:  Studies will show that there are already developed Molecular DSTs that offer an advantage over conventional phenotypic tests that are more time-consuming. Studies have it, too, that these tests can be used to diagnose TB through amplification of nucleic acids. They detect drug resistance by identifying genetic mutations in specific genes.

The Bohol Tribune (TBT):  Will you please tell us again what is meant by MDR TB in medical terms?

Dr. Bryan Cepedoza:  Ok. MDR TB means Multiple drug resistance (MDR), multidrug resistance or multiresistance is antimicrobial resistance shown by a species of microorganism to at least one antimicrobial drug in three or more antimicrobial categories.

The Bohol Tribune (TBT):  Is it curable, Dr. Bryan?

Dr. Bryan Cepedoza:  I say that usually, multidrug-resistant tuberculosis can be cured with long treatments of second-line drugs, but these are more expensive than first-line drugs and have more adverse effects. 

The Bohol Tribune (TBT):  And like what you said, it is contagious, right?

Dr. Bryan Cepedoza:  Like what I said, it is contagious. However, a contact of an MDR-TB patient if treated promptly with an effective regimen is no longer infectious after initiation of effective treatment, thus stopping transmission of this strain to others inside or outside of the home. The two reasons why MDR/RR-TB continues to emerge and spread are mismanagement of TB treatment and person-to-person transmission. Most people with TB are cured by a 6-month treatment regimen that is provided to patients with adequate support.May 20, 2024. (TO BE CONTINUED)
